The Challenge
Heart failure requires consistent monitoring between healthcare visits. Weight and blood pressure measurements, together with information about symptoms and daily observations, can provide important context for understanding changes in a patient's condition.

Project Goal
The goal of CHF Companion is to design and develop a mobile application that enables patients with congestive heart failure to monitor weight and blood pressure, automatically synchronize measurements from compatible smart devices, visualize long-term trends, maintain an observation diary, and generate structured reports for healthcare professionals.

Family members or patient assistants who support daily monitoring when a patient needs additional help using the application.

Application Prototype
The CHF Companion mobile application prototype demonstrates the core workflows for home monitoring, including health data visualization, observation tracking, reporting, and connected device management.

Project Status
CHF Companion is currently in the prototype development phase. The core system architecture, application design, and primary mobile workflows have been established, with implementation and integration work continuing toward the final Capstone release.
